WITHIN THE FRAMEWORK OF THE LAW NO. 6698 ON THE PROTECTION OF PERSONAL DATA

GENERAL INFORMATION:

In accordance with the Law No. 6698 on the Protection of Personal Data (“KVKK”), natural persons whose personal data are processed (hereinafter referred to as the “Applicant”) are granted certain rights regarding the processing of their personal data under Article 11 of the Law.

Pursuant to Article 13/1 of the KVKK, applications to be made to our Company, which is the Data Controller, concerning these rights must be submitted to us in writing or by other methods determined by the Personal Data Protection Board (“Board”).

It is important that the personal data we hold about you is accurate and up to date. Therefore, please inform us if there is any change in your personal data. Under the KVKK and related legislation, you may:

1. Learn whether your personal data is being processed,
2. Request information regarding the processing of your personal data,
3. Learn the purpose of the processing and whether it is used in accordance with that purpose, and learn the third parties to whom personal data is transferred domestically or abroad, or
4. Request correction, updating, or deletion of personal data if it is incomplete, inaccurate, or if the reasons for processing no longer exist, including notifying third parties to whom data has been transferred.

Within this scope, written applications to our company can be submitted by printing this form and writing “Request for Information under the Law on the Protection of Personal Data” on the envelope or in the subject line, via:

1. In-person submission by the applicant,
2. Notary public or registered mail with return receipt,
3. Registered electronic mail (KEP) address of our company.

8. The applicant must apply in person and provide an identity verification document.

Important Note:

Applications must be made personally. Applications cannot be made on behalf of a spouse, relative, or child. If the Company has doubts about the applicant’s identity, it may request additional verification information. If the information provided in this form is incorrect, outdated, or unauthorized, our Company assumes no responsibility for any issues arising from such misinformation or unauthorized requests.

Applications submitted to us will be evaluated according to the nature of your request and answered within 30 (thirty) days at the latest from the date of receipt, in accordance with Article 13/2 of the Law. Our response will be delivered to you in written or electronic form under Article 13/3.
